Utah Divorce Subpoena: Pharmacy Records
Sometimes, the only way to figure all the medications someone is taking is to subpoena pharmacy records.
People will doctor shop and get prescriptions from multiple doctors. This makes is difficult to track down all medical providers. Oddly, however, even people who doctor shop tend to fill all their prescriptions at one pharmacy. So, instead of trying to track down multiple doctors at multiple locations, it is many time easier to find that one pharmacy and get information there.
Pharmacy Records Request
Here is our standard pharmacy records request that we use in our Utah divorce and child custody cases:
All records, documents, correspondences, doctors’ and nurses’ notes, bills, payment histories, records of all prescriptions called in, records of all prescriptions filled, dates of pickup of prescriptions, amounts of medications prescribed and filled, and any and all other records you may have in your possession for XXXX, date of birth XXXX XX, XXXX, Social Security Number XXXX.
