Utah Divorce Subpoena: Dental Provider Records

This is the third in a series of Utah divorce subpoena request templates.

Today’s is a sample request for dental provider records. The reason you might want to subpoena dental records are numerous, but usually come down to a couple considerations:

  1. You want to prove who accompanies children to dental visits (used to prove who is the primary parent).
  2. You want to prove how much you have spent on dental care because your ex won’t pay his/her share (often used in orders to show cause).

Dental Record Request

Here’s the request:

“All records, documents, correspondences (including correspondences with third-parties regarding below-named person), call/communication logs, notes regarding communications, evaluations, doctors’ and nurses’ notes, dentists’ and assistants’ notes, raw test data, charts, test results, diagnostic studies, diagnostic films, bills, explanations of benefits, payment histories, therapy notes, pharmacy records, and all other records you may have in your possession regarding XXXX, born XXXX XX, XXXX, Social Security Number XXXX.”
